In short: PRT collects personal health information to provide clinical care. We don't sell it, we don't share it without your consent, and we protect it in accordance with Ontario's Personal Health Information Protection Act (PHIPA) and Canada's Personal Information Protection and Electronic Documents Act (PIPEDA).
Who this policy applies to
Performance Recovery Therapy (PRT) operates two clinics in Toronto: 1400 O'Connor Drive, Suite 14, East York, and 2100 Ellesmere Road, Suite 120, Scarborough. This policy applies to all patients, website visitors, and anyone who submits information to us through the site, our booking system, or in person.
What we collect
Personal and contact information
- Name, date of birth, phone number, email address, mailing address
- Emergency contact information
- Employer or occupation where relevant to treatment
Personal health information
- Medical and injury history, medications, relevant surgeries
- Clinical assessment findings, diagnoses, and treatment plans
- Session notes, imaging reports you share with us, and referral correspondence
- Consent forms and intake questionnaires
Billing information
- Insurance carrier, policy and certificate numbers, plan details
- Payment information (processed by our payment processor, not stored on our servers)
- MVA or SABS claim information where applicable
Website and technical information
- Pages visited and approximate location, collected through standard web analytics
- IP address, browser type, device type
- Information you submit through contact forms, the careers form, or our chat widget
How we use your information
- To provide and coordinate your clinical care
- To communicate with you about appointments, plans of care, and follow-ups
- To process insurance claims on your behalf when you request direct billing
- To meet our regulatory, legal, and reporting obligations as regulated health professionals
- To improve our services, train our team, and support internal quality assurance (de-identified where possible)
- To send occasional updates about PRT if you have opted in to receive them
Who we share it with
We do not sell your information. We share it only as necessary:
- Within your circle of care — with other practitioners at PRT involved in your treatment, or with external healthcare providers you authorize us to coordinate with (your family doctor, surgeon, referring specialist).
- Insurers — with your consent, for direct billing of covered services.
- Service providers — our practice management software (Jane App), payment processors, email and SMS providers, and website hosting. These providers are bound by confidentiality and data-protection obligations.
- Regulatory bodies — the College of Chiropractors of Ontario, the College of Physiotherapists of Ontario, and the College of Massage Therapists of Ontario, in the course of any audit, complaint, or investigation as required by law.
- Legal disclosures — where required by Canadian law, court order, or to prevent serious harm.
How we protect it
- Electronic records are stored in Jane App, a Canadian-hosted practice management system with encryption in transit and at rest.
- Paper records and forms are stored in locked storage at each location.
- Access is limited to staff who need the information to do their job.
- Our team is trained on PHIPA obligations and confidentiality, and we review our privacy practices regularly.
- We retain clinical records for the minimum period required by our regulatory colleges (currently ten years from last visit, or ten years after a patient's 18th birthday for minors), then securely destroy them.
Your rights
As a patient, you have the right to:
- Access your own health record, typically within 30 days of request, subject to limited legal exceptions.
- Request corrections to factual errors in your record.
- Withdraw consent for specific uses or disclosures, with the understanding that it may affect our ability to provide care or direct bill your insurer.
- Make a complaint to our Privacy Officer, to the Information and Privacy Commissioner of Ontario, or to your regulatory college.
Website cookies and analytics
This website uses Google Analytics 4 to understand how visitors use the site (which pages are popular, where visitors come from, how long they stay). IP addresses are anonymized, and we do not use this data to identify individual visitors.
When you first visit the site, a banner asks for your consent before any analytics cookies are set. Analytics is disabled by default until you accept. You can decline at any time, and the site's core functionality works identically either way. If you change your mind, clear your browser cookies for this site and the banner will reappear on your next visit.
Children
This website is not directed at children under the age of 13, and we do not knowingly collect their information through the site. Clinical information for minor patients is collected in the clinic with a parent or guardian's informed consent.
Changes to this policy
We may update this policy occasionally. The date at the top reflects the most recent revision. Material changes that affect how we handle health information will be communicated directly to active patients.
